Is this SOAP format or DAP format?
SOAP (Subjective, Objective, Assessment, Plan). This is the most widely accepted format for OT progress notes and aligns with school-based and clinical IEP documentation standards. A DAP version is on the roadmap.
Can I use this for school-based OT notes?
Yes. The template includes IEP goal progress tracking and uses neutral, objective language suitable for school-based documentation. You may need to adjust terminology to match your district's preferred format.
What age ranges does the template cover?
